بررسی دقت تشخیصی اندازه قدرت عضله چهار سر ران در ایجاد تمایز بین بیماران استئوآرتریت زانو با درجات خفیف و متوسط تا شدید علائم رادیوگرافی

چکیده

زمینه و هدف: تشخیص دقیق و تمایز ضعف عضله چهارسرران بین درجات خفیف و متوسط تا شدید استئوآرتریت زانو می­تواند به درمان به موقع و پیشگیری از عوارض بیشتر در این بیماران کمک نماید. بنابراین هدف ما از انجام این مطالعه، ارزیابی دقت اندازه قدرت عضله چهارسرران و تعیین مطلوب ترین نقطه برش آن در ایجاد تمایز بین دو گروه استئوآرتریت زانو بود.
روش بررسی: در کل 130 بیمار استئوآرتریت زانو بر پایه سیستم درجه بندی Kellgren-Lawrence (KL)، به دو گروه 65 نفره با علائم رادیوگرافی خفیف (درجه 2≥KL) و 65 بیمار با درجه علائم رادیوگرافی متوسط تا شدید (درجه 3≤KL) تقسیم بندی شدند. قدرت عضله چهارسرران با استفاده از دینانومتر ایزومتریک ارزیابی شد. دقت این اندازه توسط محاسبه حساسیت، ویژگی، سطح زیر منحنی ROC (Receiver Operating Characteristic)، نسبت درست نمایی و ارزش اخباری تعیین شد.
یافته ­ها: نتایج مطالعه­ی ما نشان داد که قدرت عضله چهارسرران حساسیت بالا و سطح خوبی از دقت را در ایجاد تمایز بین دو گروه بیماران استئوآرتریت زانو دارد. همچنین، بر پایه نتایج نسبت درست نمایی و ارزش اخباری، قدرت عضله چهارسرران به ترتیب توانایی خوبی برای خروج درجه متوسط تا شدید بیماران استئوآرتریت زانو و تشخیص دقیق بیماران درجه خفیف دارد.
نتیجه­­ گیری: یافته­های ما شواهدی برای دقت خوب قدرت عضله چهارسرران در ایجاد تمایز بین دو گروه بیماران استئوآرتریت زانو فراهم می­کند. بنابراین، پیشنهاد می­شود که این شاخص قبل از برنامه ریزی برای درمان این بیماران، مورد ارزیابی قرار گیرد.
