بررسی میزان اختلالات عملکردی تیروئیدی در بیماران پره اکلامپسی

چکیده

زمینه و هدف: اختلال پره اکلامپسی یک اختلال چند سیستمی است که با فشار خون ودفع پروتئین در نیمه دوم بارداری مشخص می­شود. عوامل متعددی از قبیل فاکتورهای ژنتیکی و محیطی در رخداد این اختلال دخیل هستند. با توجه به بررسی ای انجام شده و احتمال افزایش خطر اختلالات تیروئیدی  در مادران مبتلا به پره اکلامپسی، در این مطالعه به  بررسی میزان اختلالات عملکردی تیروئیدی در بیماران پره اکلامپسی پرداختیم.
روش بررسی: این مطالعه به صورت مقطعی اینده نگر بر روی 79 مادر مبتلا به پره اکلامپسی و 79 مادر باردار سالم که در بیمارستان امام خمینی بستری شده اند انجام شد. نمونه گیری باروش غیراحتمالی متوالی صورت گرفت. در پایان داده ها توسط آمار توصیفی(میانگین و درصد) و آمار تحلیلی (آزمونTمستقل) یا معادل غیرپارامتریک آن تحلیل شد.
یافته­ها: میانگین سن گروه بیمار 5/6±4/25 سال و در گروه سالم 6±3/29 سال بود. میانگین سن بارداری بر حسب روز در گروه بیمار 3/22±8/258 و گروه سالم 7/22±5/254 بود. آنالیز نشان می­دهد که بین سطوح هورمون های تیروئیدی دو گروه اختلاف معنی داری وجود ندارد. در بررسی های آماری بیماران با بارداری نرمال و پره اکلامپسی، به صورت معناداری آنتی تیروئید پراکسیداز((Anti TPO بالاتری  نسبت به گروه سالم داشتند ( 001/0P<).
نتیجه­گیری: با توجه به نتایج ، این مطالعه نشان می دهد که بروز پره اکلامپسی میتواند میزان اختلالات تیروییدی را تشدید کند اما این میزان معنادار نیست. لذا بهتر است مادرانی که در خطر بروز پره اکلامپسی قرار دارند تحت بررسی عملکرد تیروئید قرار گرفته و در صورت نیاز  تحت درمان های مناسب قرار بگیرند تا از تشدید این عارضه خطرناک جلوگیری شود.
