تعیین پاتوتایپ و الگوی مقاومت آنتی بیوتیکی جدایه های اشرشیاکلای مولد اسهال، جداشده از اسهال کودکان در شهرستان خرم آباد، لرستان، ایران

چکیده

زمینه و هدف :
سویه‌های بیماری‌زای اشرشیاکلای به عنوان عوامل اصلی بیماری‌های روده‌ای و خارج روده‌ای به شمار می-روند. این مطالعه با هدف شناسایی و ارزیابی پاتوتایپ غالب جدایه‌های اشرشیاکلای‌های مولد اسهال انجام شد.
مواد و روش‌ها :
در این مطالعه، تعداد 578 نمونه اسهال از کودکان زیر ده سال در خرم آباد جمع آوری شد. پس از کشت و جداسازی، جهت تعیین پاتوگروه جدایه‌های اشرشیاکلای‌های مولد اسهال، 4 پاتوتایپ اشرشیاکلای انتروتوکسیژنیک، اشرشیاکلای انتروپاتوژنیک، اشرشیاکلای انترواینویزیو، اشرشیاکلای شیگا توکسیژنیک به روش PCR بررسی شدند. حساسیت آنتی بیوتیکی جدایه‌ها به روش دیسک دیفیوژن (کربی بوئر) بررسی شد.
یافته ها:
از 578 نمونه مدفوع جدا شده از کودکان مبتلا به اسهال، 81 جدایه اشرشیا کلای مولد اسهال تشخیص داده شد، که از این میان به ترتیب فراوانی اشرشیاکلای انتروتوکسیژنیک 1/15% ، انتروپاتوژنیک 14 %، شیگا توکسیژنیک 7/9 % و انترواینویزیو 5% از کل نمونه ها بود. بیشترین مقاومت آنتی‌بیوتیکی به ترتیب در آنتی-بیوتیک‌های آموکسی‌سیلین 78%، تتراسایکلین 1/74%، کوتریماکسازول 2/64%، نالیدیکسیک‌اسید 8/56%، استرپتومایسین 9/46%، مینوسایکلین 3/38% و سیپروفلوکساسین1/16% مشاهده شد. مقاومت چند دارویی در 9/30% از این جدایه ها مشاهده شد.
نتیجه گیری :
اشرشیا کلای انتروتوکسیژنیک از متداولترین عوامل ایجاد کننده اسهال در کودکان زیر ده سال در این منطقه می باشند و لذا با مدیریت و کنترل بهداشت آب و غذا می توان از بروز چنین بیماری هایی بخصوص در فصول گرم سال جلوگیری کرد.
