بررسی ارزش تشخیصی اسید اوریک و PRO BNP در تشخیص فشارخون ریوی در بیماران اسکلرودرمی

چکیده

با توجه به اهمیت تشخیص زودهنگام هایپرتنشن شریان ریوی (PAH) در بیماران اسکلرودرمی و شناخت روش های غیرتهاجمی و در دسترس، مطالعه حاضر با هدف بررسی رابطه بین سطح سرمی اوریک اسید و pro-BNP با PAH در بیماران اسکلرودرمی انجام شد.
روش کار: مطالعه به صورت کنترل-موردی بر روی 112 بیمار مبتلا به اسکلرودرمی مراجعه کننده به بخش روماتولوژی بیمارستان گلستان اهواز در سال 1397 انجام شد. بیماران در دو گروه دارای هایپرتنشن ریوی (فشار سیستولیک شریان پولمونر بیشتر mmHg 40) (38=n) و بدون هایپرتنشن ریوی (بدون علائم PAH) (74=n) وارد مطالعه شدند. از تمام بیمارانcc 5 خون وریدی جهت ارزیابی سطوح سرمی pro-BNP و اوریک اسید گرفته شد.
نتایج: میانگین سطح سرمی اوریک اسید در گروه اسکلرودرمی با PH به طور معنادار بیشتر بود (05/0>P). اگرچه سطح سرمی pro-BNP در گروه اسکلرودرمی با PH بیشتر بود اما دو گروه از نظر آماری تفاوت معنادار نداشتند (05/0<P). Cut-off point اسید اوریک و pro-BNP به ترتیب (mg/dl) 6 و (pg/ml) 80 بدست آمد. حساسیت اسید اوریک برای تشخیص P.HTN در بیماران اسکلرودرمی برابر با 79/65%، ویژگی 13/85%، ارزش اخباری مثبت 59/71% و ارزش اخباری منفی 83/45% بدست آمد. حساسیت pro-BNP برای تشخیص P.HTN در بیماران اسکلرودرمی برابر با 68/73%، ویژگی 32/74%، ارزش اخباری مثبت 57/59% و ارزش اخباری منفی 62/84% بدست آمد.
نتیجه گیری: نتایج مطالعه حاضر نشان دهنده قابلیت کاربرد pro-BNP و اسید اوریک در غربالگری و تشخیص هایپرتنشن ریوی در بیماران مبتلا به اسکلرودرمی می باشد.
