مدیریت توان‌بخشی و درمان اختلال بلع در بیماران مبتلا به پارکینسون: یک مرور روایتی

چکیده
زمینه و هدف بیش از 80 درصد از بیماران مبتلا به پارکینسون در طول دوره بیماری خود دچار اختلال بلع (دیسفاژی) می‌شوند. اختلال بلع کیفیت زندگی را کاهش می‌دهد، مصرف دارو را پیچیده می‌کند و منجر به سوء تغذیه و پنومونی آسپیراسیون می‌شود. این اختلال یکی از دلایل اصلی مرگ در مبتلایان پارکینسون است. هدف از انجام این مطالعه، مروری بر پاتوفیزیولوژی و رویکردهای مدیریت توان‌بخشی و درمان دیسفاژی مرتبط با پارکینسون است که می‌تواند برای متخصصان مغز و اعصاب، آسیب‌شناسان گفتار و زبان و سایر پزشکانی که با بیماران مبتلا به پارکینسون مواجه هستند، مفید باشد.
روش بررسی به‌منظور دستیابی به داده‌های مورد نیاز، مقالات مرتبط با موضوع، با استفاده از جستجوی هدفمند کلیدواژه‌ها در پایگاه‌های الکترونیکی معتبر مانند Web of Science، PubMed، Google Scholar، Science Direct، SID و مگیران جمع‌آوری شدند و جهت دستیابی به مطالعات بیشتر منابع این مقالات نیز بررسی شد.
یافته‌ها براساس یافته‌های به‌دست آمده، رویکردهای مورد استفاده در مدیریت توان‌بخشی و درمان اختلال بلع در بیماران مبتلا به پارکینسون را می‌توان به زیرشاخه‌های رفتاری، تغذیه‌ای، دارویی و جراحی و توان‌بخشی تقسیم کرد و با توجه به شرایط بیمار از یک یا چند رویکرد به‌طور توأمان بهره برد.
نتیجه‌گیری درنهایت، به نظر می‌رسد به‌کارگیری یک رویکرد تیمی چندرشته‌ای برای شناسایی و مدیریت صحیح دیسفاژی در بیماران پارکینسون حائز اهمیت بوده و غربالگری‌های بالینی منظم همراه‌ با ارزیابی‌های ابزاری عینی برای تشخیص زودهنگام دیسفاژی ضروری است. به‌منظور تعیین اثربخشی درمان‌های مختلف بر دیسفاژی بیماران مبتلا به پارکینسون، انجام تحقیقات بیشتر به‌ویژه مطالعات آینده‌نگر و کارآزمایی‌های تصادفی‌ و کنترل‌شده مورد نیاز است.
