Neuromuscular control is believed to be a critical factor in dynamic knee stability in ACL deficient (ACLD) patients. Neuromuscular training programs (in particular perturbation training) are increasingly integrated into clinical practice for ACL lesion rehabilitation. The purpose of this study was to investigate the effect of the modified perturbation training on neuromuscular control system and functional improvement in ACLD. Subjects and Methods: 10 professional male athletes with  an average of 6.7±3.19 months after their unilateral ACL rupture participated in this study. Surface Electromyographic (EMG) data were recorded during a cross hop task, from the rectus femoris, vastus medialis, medial head of the gastrocnemius, biceps femoris and gluteus maximus muscles. Muscle activation patterns before and after 10 sessions of perturbation training were compared. Treatment outcome was determined from scores of questionnaires and functional tests. Results: Scores of IKDC subjective questionnaire and functional tests were significantly improved (P<0.05). Muscle activation patterns were modified. The significant earlier onset and late peak of the rectus femoris resulted in the longer duration from onset-to-peak activity in both of the involved and non involved limbs (P<0.05). Conclusion: Perturbation training has a central effect that modifies neuromuscular control system through the change in feed-forward control for ACL deficient patients. Rectus femoris activation is not harmful to ACL and causes a protective effect. Optimum activity of this muscle is important for ACL injury prevention and rehabilitation. Sci Med J 2012;10(6):615-627}, keywords = {Anterior cruciate ligament deficiency,Electromy- ography,Muscle activation pattern,Perturbation training,Neuromuscular control}, title_fa = {تأثیر پروتکل تعدیل‌شده درمان اغتشاشی بر الگوی فعالیت عضلانی و عملکرد بیماران مبتلا به پارگی لیگامان متقاطع قدامی}, abstract_fa = {زمینه و هدف: پارگی لیگامان متقاطع قدامی (ACL) از عمده آسیب­های مفصل زانو است. کنترل عصبی- عضلانی فاکتور حیاتی در ثبات دینامیکی مفصل زانوی بیماران دارای پارگی لیگامان متقاطع قدامی (ACLD)، می­باشد. آموزش      عصبی- عضلانی (به­ویژه اغتشاش درمانی) در تمرین درمانی و توانبخشی آسیب ACL افزایش یافته است. هدف از این تحقیق بررسی تأثیر پروتکل تعدیل­شدۀ اغتشاش درمانی بر سیستم کنترل عصبی- عضلانی و بهبود عملکرد بیماران ACLD می­باشد. روش بررسی: 10 ورزشکار حرفه­ای مرد، با میانگین گذشت 19/3 ±7/6 ماه از پارگی یک­طرفه ACL شرکت داده شدند. اطلاعات الکترومایوگرافی سطحی(EMG) از عضلات رکتوس فموریس، واستوس مدیالیس، مدیال گاستروکنمیوس، لترال همسترینگ و گلتئوس ماگزیموس، طی انجام پرش متقاطع، جمع­آوری شد. الگوی فعالیت عضلات قبل و بعد از 10 جلسه اغتشاش درمانی مقایسه شد. نتیجة درمان از نمرات پرسش­نامه­ها و  آزمون­های عملکردی مشخص شد. یافته­ها: نمرات پرسش­نامه IKDC و آزمون­های عملکردی بهبود معناداری داشتند(05/0>P). الگوی فعالیت عضلات تعدیل گردید. عضلة رکتوس فموریس در هر دو اندام درگیر و غیردرگیر، از یک سو شروع فعالیت زود هنگام­تر و از سوی دیگر زمان به قله رسیدن دیرتر و در مجموع فاصلة زمانی وارد عمل شدن تا رسیدن به قلة فعالیت طولانی­تری را نشان داد (05/0>P ). نتیجه­گیری: اغتشاش درمانی تأثیر مرکزی بر سیستم کنترل عصبی-عضلانی داشته و با تغییر در کنترل فیدفوروارد (feedforward) این سیستم، سبب تعدیل آن در بیماران ACLD می­شود. فعالیت عضلة رکتوس فموریس، نه تنها به زیان ACL نیست، بلکه نقش حمایتی دارد. فعالیت بهینة این عضله در پیش­گیری و توانبخشی آسیب  ACLمهم می باشد. م ع پ 1390؛‌10(6): 615- 627}, keywords_fa = {پارگی لیگامان متقاطع قدامی,الکترومایوگرافی,الگوی فعالیت عضلانی,اغتشاش درمانی,کنترل عصبی-عضلانی}, url = {https://jsmj.ajums.ac.ir/article_55112.html}, eprint = {https://jsmj.ajums.ac.ir/article_55112_8d5fc738936ba41fcc207fb25de3a977.pdf} }
