مقایسه پهنای سینوس کرونر بیماران مبتلا به تنگی عروق کرونر با افراد سالم به وسیله اکوکاردیوگرافی ترانس توراسیک

چکیده

زمینه و هدف: در بیماران مبتلا به بیماری عروق کرونر، به علت تنگی هرکدام از شریان های کرونر، میزان جریان خون ورید های مربوطه کاهش یافته و در نتیجه میزان جریان خون ورید سینوس کرونر نیز کاهش می‌یابد. روش‌های استاندارد اندازه گیری جریان خون ورید سینوس کرونر تهاجمی است و نیاز به انجام کاتتریسم یا استفاده از مواد پرتوزا دارد. از مدتی پیش اکوکاردیوگرافی ترانس توراسیک برای بررسی میزان جریان خون سینوس کرونر به کار گرفته شده است که به دلیل مشکلات در اندازه گیری پارامترهای مربوطه و احتمال خطا در اندازه گیری، سعی نمودیم در مطالعه‌ی حاضر وجود رابطه بین پهنای سینوس کرونر را با تنگی شرایین کرونر بررسی کنیم.
روش بررسی: در یک مطالعه‌ی مقطعی بر روی مراجعه کنندگان به بخش آنژیوگرافی 36 نفر که واجد شرایط بودند وارد مطالعه شدند. پهنای سینوس کرونر یک روز قبل از آنژیوگرافی به وسیله‌ی متخصص قلب و عروق با روش اکوکاردیوگرافی ترانس توراسیک اندازه گیری شد و سپس نتایج آنژیوگرافی این بیماران با نتایج اکو مقایسه شد.
یافته‌ها: با مقایسه‌ی نتایج اکو و آنژیوگرافی، بین پهنای سینوس کرونر و وجود تنگی قابل توجه در شرایین کرونر، رابطه‌ی معنی دار به دست آمد (P=0.001) ضمنا بین تعداد رگ‌های درگیر با پهنای سینوس کرونر نیز رابطه‌ی معنی دار مشاهده شد.
نتیجه گیری: به نظر می‌رسد می توان پهنای سینوس کرونر در اکوکاردیوگرافی (که روشی در دسترس، غیر تهاجمی، بدون عارضه و ارزان است) را به عنوان نشانه‌ای از وجود تنگی قابل توجه در شرایین کرونر محسوب کرد.
