استفاده از روش‌های اینترونشنال در درمان کودکان کمتر از 15 سال مبتلا به باز بودن مجرای شریانی در بیمارستان شهید چمران اصفهان

چکیده

زمینهوهدف: باز بودن مجرای شریانی (Patent Ductus Arteriosus)یا PDA یکی از بیماری­های مادرزادی قلبی است که حدود 10%-8% از بیماری های قلبی مادرزادی را تشکیل می­دهد. باز بودن این مجرا بعداز تولد می­تواند در مواردی باعث نارسایی قلبی و یا اندارتریت گردد. به همین علت بستن این مجرا با روش­های جراحی یا اینترونشن توصیه می­گردد. هدف از این مطالعه بررسی اثربخشی استفاده از روش های اینترونشن در بستن PDA در مرکز شهید چمران دانشگاه علوم پزشکی اصفهان بود.
روشبررسی: 39 بیمار که 18 نفر پسر و 21 نفر دختر بود و PDA  در آنها توسط اکوکاردیوگرافی تشخیص داده شده بود در این مطالعه بررسی شدند.
یافته­ها: متوسط وزن بیماران 43/6±82/10 کیلوگرم بود. متوسط سن بیماران  8/11±3/14 ماه بود. در 15  بیمار کویل و در 24 بیمار امپلاتزر جهت بستن PDA استفاده شد. 10 نفر از این بیماران کویل Cook  و 5 نفر کویل Pfm  استفاده شد. میانگین اندازه PDA در تنگترین قسمت متصل به شریان پولمونر 7/1±2/3 میلیمتر بود. میانگین زمان آنژیوگرافی  2/12±3/45 دقیقه و زمان فلوروسکوپی 3/6±5/8 دقیقه بود. فشار سیستول و دیاستول شریان ریوی بترتیب 4/8±3/34 و 2/6±2/14 میلیمترجیوه بود. در بیماران ما عوارض حاد مانند مرگ، خونریزی شدید، پارگی عروق یا آریتمی دیده نشد. عوارض دیرس مانند بروز تنگی در آئورت و یا پولمونر، جابجایی وسیله، شکستگی و یا تغییر شکل وسیله و یا وجود نشت دیده نشد.
نتیجه­گیری: در تجربه ما در این مرکز بی خطر بودن استفاده از روش­های اینترونشن جهت بستن PDA نشان داده شد.
