اثربخشی نگهداشتن ورید آزیگوس در بیماران آترزی مری دارای فیستول مری-تراشه

چکیده

مقدمه
آترزی مری (Esophageal atresia (EA)) از شایع ترین آنومالی های مادرزادی در نوزادان بوده و در هر 4500-3000 تولد زنده یک مورد آترزی مری اتفاق می افتد. علت بروز آترزی مر و فیستول های تراکئوازوفاژیال(tracheo-esophageal fistula(TEF)) مشخص نمی باشد. بطور طبیعی این دولوله تا روز 32 حاملگی از یکدیگر جدا می گردند. چنانچه عمل جدا شدن به صورت ناقص صورت پذیرد، طیف وسیعی از مالفورماسیون ها به وجود خواهند آمد. آترزی مری همراه با فیستول تراشه به مری یکی از شایع ترین عمل های جراحی اورژانس در نوزادان می باشد.(1-4)
آنومالی های همراه در نوزادان مبتلا به آترزی مری شایع بوده و تقریبا 50 درصد این نوزادان مبتلا به آنومالی در دیگر ارگان ها می باشند. آترزی مری به عنوان بخشی از مجموعه نواقصی از قبیل نواقص ستون فقرات، آنورکتال، قلب، تراکوازوفاژیال، کلیوی و اندام ها می باشد. به طور کلی آترزی مری را می توان به دو دسته شایع و غیرشایع تقسیم بندی کرد که نوع شایع 85 درصد موارد را شامل می شود و حالتی می باشد که همراه با آترزی مری یک فیستول بین قسمت دیستال مری و تراشه دیده می شود. تقریبا در 20 درصد نوزادان متولد شده با آترزی مری بعضی از اشکال بیماری های مادرزادی قلب مشاهده می شود. همراهی بیماری های مادرزادی قلب با آنومالی های دستگاه گوارش می تواند باعث افزایش مورتالیتی و موبیدیتی
