سندروم گلدنهار
سندروم گلدنهار یا سندروم چشمی-گوشی- مهرهای، بیماری نادری است که با ناهنجاریهای متعدد کرانیوفاسیال نظیر: هیپوپلازی ماندیبول، استخوان گونه، گوش و آنومالیهای ستون مهرهای مشخص میشود. علت این سندروم به درستی روشن نیست، ولی گوناگون بهنظر میرسد و در برخی از موارد نیز ارثی جلوه میکند. در اینجا دختری سه ماهه مبتلا به این سندروم گزارش میشود. او برای معاینة دورهای مراجعه کرده بود. در معاینه، یافتههای غیرطبیعی عبارت بودند از: نامتقارن بودن چهره، گشادی یک طرف دهان، منگولة جلوی گوش، پایین نشستن گوش، آترزی مجرای گوش و کیست درموئید اپیبولبر که همگی منحصراً در طرف چپ مشاهده شدند. آزمون شنوایی پاسخ ساقة مغز (ای بی آر) نیز کری هدایتی شدید تا عمیق گوش چپ را نشان داد. بیمار، برادری بزرگتر از خود دارد که همین علایم در او نیز وجود داشت.
ناهنجاریهای مادرزادی در سندروم گلدنهار یک طرفهاند. تشخیص زودهنگام کاهش شنوایی در تکامل بیمار بسیار با اهمیت است.

ادنتوژنیک کراتوسیست با نمای ضایعه پریآپیکال
زمینه و هدف: ادنتوژنیک کراتوسیست نوع مشخصی از کیست ادنتوژنیک تکاملی است که بایستی بهعلت ویژگیهای هیستوپاتولوژیک و مشی بالینی خاص مورد توجه قرار گیرد. این کیست دارای رفتار تهاجمی و تمایل به عود بالا بوده و تحت عنوان تومور خوشخیم سیستیک در نظر گرفته میشود و دارای نمای رادیوگرافیکی متغیر بوده که از رادیولوسنسی تک حفرهای تا چند حفرهای متفاوت است. گاهی بهصورت بین ریشهای قرار گرفته و در موارد نادرتر بهصورت رادیولوسنسی پریآپیکال بوده و ممکن است با ضایعات اندودنتیک اشتباه شود و در نتیجه به تشخیص و درمان نامناسب منجر گردد.
گزارش مورد: مردی 33 ساله بهدلیل تورم در قدام فک پایین مراجعه نمود که با درمان ریشة دندان لترال سمت راست بهبود نیافته بود. در بررسی هیستوپاتولوژیکی ضایعة رادیولوسنت خارج شده از فک، ادنتوژنیک کراتوسیست ملتهب گزارش شد.
نتیجهگیری: تشخیص تنها بر پایة یافتههای بالینی میتواند مشکل آفرین باشد، زیرا میتواند به درمان نامناسب منجر شود. در اکثر موارد، بررسی هیستولوژیک بافتهای خارجشده توسط جراحی برای تشخیص صحیح الزامی است. این مورد بهویژه وقتی صادق است که ضایعه با درمانهای معمول ریشة دندان برطرف و درمان نشود.

ارزیابی کفایت دیالیز بهوسیلةKT/V در بیماران همودیالیز
زمینه و هدف: بیماران مبتلا به نارسایی مرحلة پایانی کلیه، نیازمند دریافت دیالیز با کفایت کافی میباشند. بر اساس خطمشیهای DOQI حداقل کفایت دیالیز به روشKT/V در بیماران همودیالیزی 2/1 یا بیشتر است. در این مطالعه، هدف ما تعیین کفایت دیالیز در بیماران همودیالیزی بیمارستان شهید بهشتی آبادان میباشد. روش بررسی: ما در مطالعة مقطعی و آیندهنگر، در تابستان سال 1389 مقادیرKT/V بیماران همودیالیزی آبادان را ارزیابی نمودیم. همودیالیز بهمدت 3 تا 4 ساعت با صافی سنتتیک و با مایع دیالیز بیکربناتی انجام میشد. بلافاصله قبل و بعد از هر دیالیز، نیتروژن اورة خون (BUN) اندازهگیری میشد. برای BUN بعد از دیالیز، سرعت جریان خون به 100 سیسی در دقیقه کاهش و پس از 15 ثانیه نمونة خون گرفته میشد. برای تخمینKT/V از درصد کاهش اوره (PRU) از فرمول زیر استفاده نمودیم. 460/0- (PRU * 026/0) KT/V = یافتهها: 54 بیمار همودیالیزی (28 زن و 26 مرد) با سن متوسط 2/14± 39 سال در مطالعه قرار داده شدند. شایعترین علت ESRD فشار خون (07/24 درصد)، ناشناخته (22/22 درصد)، دیابت (51/18 درصد)، گلومرولونفریت مزمن (81/14 درصد)، انسداد مجاری ادراری (96/12 درصد) و بیماری کیستیک کلیه (40/7 درصد) بود. KT/Vدر03/87 بیماران (74 نفر) کمتر از 2/1 محاسبه شد. تفاوت قابل توجهی میان مقدار KT/V در مرد و زن (57/0P=) و غلظتهای مختلف هموگلوبین (58/0P =) وجود نداشت. نتیجهگیری: نتایج این مطالعه نشان داد که بیشتر بیماران همودیالیزی این مرکز، حداقل دوز دیالیز را دریافت ننمودهاند و ما باید علل آن را ارزیابی و اصلاح نماییم.

اعتبارسنجی مقیاس RPE 6-20 بورگ بر اساس ضربان قلب در کارگران مرد بخش صنعت شهرستان شیراز
زمینه و هدف: یکی از روشهای ارزیابی و برآورد بار کار جسمانی، روش ارزیابی ذهنی است. در سالهای اخیر ارزیابی تلاش درک شده یا علایم ذهنی خستگی حین کار فیزیکی، تقریبا بر روشهای اندازهگیری روانی- فیزیکی، متکی شده است. مقیاسهای نرخگذاری، رایجترین ابزار اندازهگیری بهکار برده شده در روانشناسی هستند. نرخگذاری تلاش درک شدة بورگ (RPE) یک روش آسان برای پایش شدت فعالیت فیزیکی است. این مطالعه با هدف اعتبارسنجی همبستگی بین RPE و ضربان قلب در بین کارگران ایرانی انجام شده است.
روش بررسی: در این مطالعه، 500 نفر از کارگران مرد سالم کارخانجات شهرستان شیراز داوطلبانه شرکت نمودند (گسترة سن20 تا 59 سال). افراد مورد مطالعه به وسیلة دوچرخة ارگومتر براساس پروتکل آستراند بهمدت 6 دقیقه مورد ارزیابی قرار گرفتند. از این افراد حین انجام آزمایش خواسته شد تا مقیاس بورگ را در پایان هر دقیقه نرخگذاری کنند و همچنین ضربان قلب آنان در پایان هر دقیقه توسط دستگاه ثبت شد.
یافتهها: ضریب همبستگی پیرسون (r) بین RPE و ضربان قلب بهدست آمده از تمامی دادهها، معادل 847/0 بهدست آمد که نشاندهندة همبستگی بالاست.
نتیجهگیری: آنالیز دادهها نشان داد که رابطة بین RPE و ضربان قلب بالا بوده و مؤید نتایج بهدست آمده از مطالعات پیشین میباشد. بنابراین، میتوان از مقیاس RPE بورگ برای ارزیابی شدت فعالیت در کارگران مرد ایرانی سالم استفاده نمود.

مقایسة اثر افزودن الانزاپین در مقابل پرفنازین بر درمان نگهدارندة بیماران دو قطبی نوع یک
زمینه و هدف: اختلال دو قطبی از شایعترین اختلالات روانپزشکی است. میزان شیوع آن در کل جمعیت، یک درصد میباشد. این اختلال بهطور شایع در دوران جوانی شروع میشود. 25 تا 50 درصد بیماران حداقل یک بار برای خودکشی تلاش جدی کردهاند. با توجه به لزوم درمان نگهدارنده و کاهش هزینهها برای بیمار، این مطالعه به مقایسة اثر داروی پرفنازین در مقابل الانزاپین در درمان نگهدارنده بیماران میپردازد.
روش بررسی: این مطالعه از نوع مداخلهای میباشد که روی 40 نفر از بیماران مبتلا به اختلال دو قطبی نوع یک 16 تا 50 سال که بر روی درمان نگهدارنده بودند، بهمدت 6 هفته انجام شد. این بیماران در سال 1389 به درمانگاه اعصاب و روان بیمارستان گلستان اهواز مراجعه کرده بودند. شدت علایم با استفاده از معیار نمرهدهی مانیای یانگ در بدو ورود و هفتههای 3 و6 مورد ارزیابی و مقایسه قرار گرفت. دادهها توسط نرمافزار SPSS تجزیه و تحلیل شدند.
یافتهها: در هر دو گروه پرفنازین و الانزاپین علایم در انتهای مطالعه نسبت به ابتدای مطالعه تفاوت معنادار داشته است (000/0P= برای الانزاپین و001/0 P= برای پرفنازین). میزان بهبودی بین دو گروه تفاوت معناداری نداشته است.
نتیجهگیری: میزان بهبودی بین دو گروه تفاوت معناداری نداشته است. که در تفسیر آن میتوان به دورة کوتاه مطالعه و نمونة کم و استفاده از آنتیسایکوز در کنار تثبیتکنندة خلق اشاره کرد.

بررسی تأثیر آموزش ایمنسازی در مقابل استرس بر افسردگی و کیفیت زندگی بیماران مبتلا به MS
زمینه و هدف: هدف از این پژوهش، بررسی تأثیر آموزش ایمنسازی در مقابل استرس بر افسردگی و کیفیت زندگی بیماران مبتلا بهMS بوده است.
روش بررسی: پژوهش حاضر از نوع آزمایشی با طرح پیشآزمون- پسآزمون و گروه کنترل است. جامعة آماری شامل 83 نفر از بیماران مبتلا به MS شهر اصفهان میباشد. برای انتخاب این افراد از روش نمونهگیری داوطلبانه در دسترس و همتاسازی استفاده شد. آنها از لحاظ سن، جنس، وضعیت اقتصادی- اجتماعی، طول مدت ابتلا به بیماری، دارا بودن یک انحراف معیار بالاتر از میانگین در افسردگی و سایر متغیرهای مورد نظر در پژوهش کاملاً همگون بودند. سپس با استفاده از روش تصادفی 30 نفر انتخاب و بهصورت تصادفی در دو گروه آزمایش و گواه قرار گرفتند. ابزار مورد استفاده در پژوهش شامل پرسشنامة دموگرافیک، پرسشنامة افسردگی بک (BDI) و مقیاس کیفیت زندگی مخصوص بیماران MS (MSIS-29) بوده است. آزمودنیهای گروه آزمایش بهمدت 8 جلسة دو ساعته، تحت درمان آموزش ایمنسازی در مقابل استرس (SIT) قرار گرفتند، اما گروه کنترل مداخلهای دریافت نکردند. نتایج حاصل با استفاده از تحلیل واریانس چندمتغیری (MANOVA) تجزیه و تحلیل شدند.
یافتهها: نتایج حاصل آشکار ساخت که میان گروه آزمایش و گواه از لحاظ افسردگی و کیفیت زندگی، تفاوت معناداری وجود دارد (001/0P<). یعنی آموزش ایمنسازی در مقابل استرس موجب کاهش افسردگی و افزایش کیفیت زندگی بیماران مبتلا به MS شده است.
نتیجهگیری: آموزش ایمنسازی در مقابل استرس بهطور معناداری موجب کاهش میزان افسردگی و افزایش کیفیت زندگی بیماران MSشده است. بنابراین آموزش این مهارتها و راهبردهای مقابله با استرس، علاوه بر درمانهای دارویی به این بیماران توصیه میشود.

بررسی میزان کارآیی روش Real-Time PCR با استفاده از دو راهکار رسم منحنی استاندارد و رگرسیون خطی
زمینه و هدف: آنالیز کمّیmRNA با استفاده از Real-Time PCR بهوسیله روش سایبرگرین بهطور وسیعی در مطالعات بیولوژیک کاربرد بسیاری پیدا کرده است. هدف این تحقیق مقایسة کارآییPCR محاسبه شده با استفاده از دو روش شیب خط نمودار استاندارد و نرمافزارLinRegPCR جهت تعیین استراتژی بهتر برای محاسبة کارآیی PCR میباشد.
روش بررسی: در این پژوهش پس از خونگیری و استخراج RNA و سنتز cDNAفرآیند qRT-PCR انجام شد.سپس محاسبة PCR Efficiency به دو روش رسم منحنی استاندارد و همچنین با استفاده از نرمافزارLinReg PCR انجام گرفت و در نهایت این دو روش آنالیز با هم مقایسه شدند.
یافتهها: میزان کارآیی PCR محاسبه شده برای سه ژنGAPDH، TGF βو IL-10 با روش منحنی استاندارد بهترتیب 99/1، 81/1 و 87/1 و همچنین با روش استفاده از نرمافزار رایانهای LinReg PCR کارآیی این سه ژن بهترتیب 98/1، 82/1 و82/1 بهدست آمد.
نتیجهگیری: نتایج این تحقیق نشان میدهد که در آنالیز دادههای حاصل از تکثیر cDNA میزان کارآییPCR بهدست آمده برای سه ژن GAPDH، TGF β و IL-10با دو روش مختلف منحنی استاندارد و LineRegPCR نتایج تقریباً یکسانی حاصل میشود. بنابراین، توصیه میشود که بهجای روش پرهزینه منحنی استاندارد از نرمافزار LinReg PCR استفاده گردد.

بررسی اثر جنیستئین به همراه محدودیت کالری بر اصلاح پروفایل لیپیدی، گلوکز و فاکتورهای التهابی در موشهای چاقشده با رژیم پرچرب
زمینه و هدف: چاقی یکی از بزرگترین مشکلات بهداشت عمومی است که موجب اختلالاتی از جمله دیابت، دیسلیپیدمی، بیماریهای عروق کرونر و برخی انواع سرطانها میشود. بهنظر میرسد ایزوفلاوونهای سویا میتوانند بر بهبود چاقی و کاهش عوارض ناشی از آن مؤثر باشند. در این مطالعه، اثر تجویز جنیستئین به همراه محدودیت کالری بر موشهای چاقشده با رژیم پرچرب مورد بررسی قرار گرفت.
روش بررسی: 30 سر موش نر چاقشده با رژیم پرچرب بهطور تصادفی به 3 گروه 10 تایی به شرح زیر تقسیم شدند: 1) گروه موشهای چاق دریافتکنندة محدودیت کالری بههمراه mg/kgbw 50 جنیستئین 2) گروه موشهای چاق دریافتکنندة محدودیت کالری بههمراه Dimethyl Sulphoxide (DMSO) (شاهد 1 و 3) گروه موشهای چاق دریافتکنندة غذای استاندارد جوندگان به میزان آزاد (شاهد 2). پس از 4 هفته نمونههای خون در حالت ناشتا جهت بررسی آزمایشگاهی جمعآوری گردید.
یافتهها: تجویز جنیستئین بههمراه محدودیت کالری موجب تشدید بهبود غلظت تریگلیسیرید (005/0P=)، VLDL (005/0P=)، کلسترول تام (002/0P=) وLDL-C (003/0P=) شده و HDL-C (001/0P=) را نیز افزایش میدهد، ولی بر وزن، فاکتورهای پیشالتهابی رزیستین و C-Reactive Protein (CRP) بیتأثیر بود. محدودیت کالری نیز موجب کاهش معنادار گلوکز، تریگلیسیرید، کلسترول تام، LDL-C، CRP و افزایش HDL-C میشود (05/0>p)، ولی بر روی غلظت سرمی رزیستین بیتأثیر بود.
نتیجهگیری: بهنظر میرسد که ترکیب محدودیت کالری بههمراه ایزوفلاوونهای سویا روشی مؤثر در بهبود پروفایل لیپیدی در افراد هایپرلیپیدمیک میباشد.

اثرات تمرین ورزشی اجباری بر علایم سندرم محرومیت، تعداد نورونهای هیپوکامپ مغز و میزان هورمون کورتیکوسترون پلاسما در موشهای صحرایی نر معتاد به مورفین
زمینه و هدف: اعتیاد به مورفین موجب اختلال در اعمال رفتاری- شناختی میگردد. هدف از مطالعة حاضر بررسی اثرات تمرین ورزشی اجباری درون تریدمیل بر علایم محرومیت از مورفین، میزان ترشح هورمون کورتیکوسترون، و شمارش تعداد نورونهای هیپوکامپی در نیمکرههای مغز حیوانات معتاد به مورفین بود.
روش بررسی: موشهای صحرایی نر بالغ نژاد ویستار در پنج گروه دهتایی شامل: کنترل تمرین کرده، کنترل شاهد تمرین، معتاد، معتاد تمرین کرده و معتاد شاهد تمرین آزمایش شدند. در گروههای معتاد به مورفین با تجویز نالوکسان علایم سندرم محرومیت بهمدت 30 دقیقه شمارش شدند. حیوانات در گروههای تمرینکردة روزانه بهمدت یک ساعت برای 10 روز متوالی (ساعت 10- 9 صبح) درون تریدمیل دویدند. گروههای شاهد تمرین، مدت مشابهی را درون تریدمیل با موتور خاموش ولی با شوک روشن قرار داشتند. در پایان آزمایشات میزان کورتیکوسترون سرم و شمارش نورونهای هیپوکامپ انجام گردید. از روش آماری آنالیز واریانس یکطرفه و تست پشتیبانی LSD برای آنالیز یافتهها استفاده شد. تفاوت بین نتایج با 05/0 P< معنادار تلقی گردید.
یافتهها: اعتیاد به مورفین موجب افزایش علایم سندروم محرومیت، افزایش ترشح هورمون کورتیکوسترون، و کاهش تعداد نورونهای هیپوکامپی در هر دو نیمکرههای مغزی گردید که همه معنادار بودند. تمرین اجباری علایم سندرم را بهطور معناداری کاهش داد، اما نتوانست میزان ترشح کورتیکوسترون و تعداد کاهش یافتة نورونهای هیپوکامپ را بهطور کامل طبیعی نماید.
نتیجهگیری: تمرین ورزشی اجباری علیرغم اثرات سودمندش بر سلامت بدن و بهویژه شناخت، لیکن در موشهای معتاد به مورفین بهعلت ایجاد استرس اکسیداتیو و عدم جبران تغییر ترشح برخی هورمونها (مانند کورتیکوتروپینها) متعاقب اعتیاد، تأثیرش کمتر میگردد.

بررسی اختلال عملکرد تیروئید و آنتیبادیهای ضد تیروئید در بیماران دیابت I
زمینه و هدف: برای ارزیابی ارتباط بین اختلال عملکرد تیروئید و خود ایمنی تیروئید با دیابت ملیتوس یک نمونه تصادفی از 146بیمار دیابت نوع 1 را مورد مطالعه قرار دادیم.
روش بررسی: تمام بیماران دیابتی ارجاع شده به کلینیک دیابت مرکز تحقیقات دیابت دانشگاه علوم پزشکی جندیشاپور اهواز از نظر سطح Anti-TG Ab، Anti – Tpo Ab، T3 و T4 و TSH ، T3Ru و Hb A1C مورد سنجش قرار گرفتند. برای هر بیمار پرسشنامهای شامل: زمان ابتلا، سن، جنس، داروهای مصرفی پایین آورندة قند خون جهت ارتباط سنجی بین اختلال عملکرد تیروئید و هر کدام از موارد ذکر شده پر شد.
یافتهها: 7/37 درصد از بیماران اختلال عملکرد تیروئید داشتند که اختلالات عملکرد تیروئید به ترتیب شیوع ساب کلینیکال هیپوتیروئیدی 13 درصد، هیپوتیروئیدی 9/8 درصد، هیپرتیروئیدی 9/8 درصد و ساب کلینیکال هیپرتیروئیدی 8/6 درصد بود. 40 درصد از زنان و 30 درصد مردان اختلال عملکرد تیروئید داشتند (045/0= P) و هر چه بیماران مسنتر بودند شیوع اختلال عملکرد در آنها بیشتر بود (001/0=P). بین طول مدت ابتلا به دیابت و اختلال فانکشن تیروئید هم ارتباط معناداری وجود داشت (05/0P<) اما بین HbA1c با اختلال عملکرد تیروئید ارتباط معناداری بهدست نیامد.
شیوع سطوح بالای Ant-Tpo Ab و Anti-TG Ab بهترتیب در 60 درصد و 9/42 درصد از بیماران دیده شد.
نتیجهگیری: توصیه میشود که همه بیماران دیابتی خصوصاً زنان و افراد مسنتر و آنها که طول مدت دیابت طولانیتری دارند بهطور سالیانه از نظر اختلال عملکرد تیروئید مورد ارزیابی قرار گیرند.

ارتباط یافتههای سنتیگرافی کلیه با یافتههای بالینی و آزمایشگاهی در کودکان مبتلا به عفونت تبدار مجاری ادراری
زمینه و هدف: عفونت مجاری ادراری یکی از بیماریهای شایع و مهم کودکان است. علایم غیراختصاصی این بیماری در دوران شیرخوارگی و در کودکان خردسال، تشخیص عفونت ادراری فوقانی از تحتانی را مشکل میسازد. هدف از این مطالعه، بررسی رابطه بین یافتههای پیلونفریت در اسکن کلیه با یافتههای بالینی و آزمایشگاهی در کودکان مبتلا به عفونت ادراری تبدار بوده است.
روش بررسی: این مطالعة آیندهنگر بر روی 83 شیرخوار و کودک یک ماه تا 8 ساله که بهدنبال عفونت ادراری تبدار در بخش کلیة بیمارستان کودکان ابوذر بستری شده بودند، انجام شد. در 5 روز ابتدایی بستری بیماران، اسکن کورتکس کلیه با DMSA نشاندار شده با تکنسیوم 99، سونوگرافی، VCUG، ESR، CRP، هموگلوبین، تعداد گویچههای سفید خون و آنالیز کامل ادرار انجام شد.
یافتهها: میانگین سنی بیماران ما 3/24 ماه و 82 درصد (68 نفر) از آنان دختر بودند. اسکن کلیه در 45 نفر از بیماران (2/54 درصد) علایم پیلونفریت حاد را نشان داد که سن متوسط آنان 2/30 ماه و شامل 9 پسر (20 درصد) و 36 دختر (80 درصد) بودند. مدت تب، میزان تب، لکوسیتوز، کمخونی، پروتئینوری، ESR بالا و CRP مثبت، رابطة آماری معناداری با یافتههای پیلونفریت حاد در اسکن کلیه داشتند (05/0p<). برگشت ادرار به حالبها در 5/20 درصد از بیماران مشاهده شد که رابطة معناداری با یافتههای پیلونفریت حاد در اسکن کلیه نداشت.
نتیجهگیری: گرچهبا انجام اسکن کورتکس کلیه در طی ابتلا به عفونت تبدار مجاری ادراری میتوان درگیری کلیه و قطب مبتلای آن را مشخص نمود، برخی از یافتههای بالینی و آزمایشگاهی قادرند ایجاد علایم پیلونفریت را در اسکن کلیه پیشگویی کنند و نیاز به بررسیهای بیشتر برای تشخیص عوارض احتمالی در آینده را مطرح نمایند.

مقایسة لابراتواری میزان سختی یک نوع کامپوزیت Core، در دو روش مختلف قراردهی
زمینه و هدف: هدف از این مطالعه، مقایسة لابراتواری میزان سختی یک نوع کامپوزیت کور (Core) در بازسازی کامل دندان، در روشهای قراردهی قطعهایو تودهای(Bulk) بود.
روش بررسی: ده نمونة استوانهای از کامپوزیت Clearfil Photo Core-Iight Cure (Kuraray/Japan) که ترنسلوسنت و تکرنگ است، در سرنگهای انسولین تا ارتفاع نه میلیمتر تهیه شد. نیمی از آنها با روش قرار دهی قطعهای با قطعات دو میلیمتری و نیمی از آنها با روش قراردهی تودهای ساخته شد. نوردهی با شدت mw/cm2650 و بهمدت 40 ثانیه بود. نمونهها به طور طولی برش خورده و در اپوکسی رزین مانت شدند. میکروسختی ویکرز از سطح هر نمونه در عمقهای 1 تا 9 میلیمتری اندازهگیری شد.
یافتهها: میزان سختی عمقهای معادل، در روش قراردهی تودهای بیشتر از روش قراردهی قطعهای بود. در هر دو روش قراردهی، تفاوت معناداری در سختی بین عمقهای یک تا نه میلیمتری نبود.
نتیجهگیری: جهت دستیابی به عمق کیورینگ مناسب در هر کامپوزیت، با توجه به ترکیب و رنگ آن، باید شرایط نوردهی استاندارد آن کامپوزیت خاص را رعایت نمود.
