بررسی تأثیر کاشت حلزون بر شکل پذیری قشر شنوایی در کودکان با کاهش شنوایی مادرزادی
زمینه و هدف: امروزه استفاده از کاشت حلزون یک روش درمانی مفید در درمان کودکان با کاهش شنوایی عمیق محسوب می شود. پتانسیل های برانگیخته شنوایی قشری (CAEPs) از جمله روش های غیرتهاجمی می باشند که می توانند برای ارزیابی آبجکتیو تکامل قشر شنوایی در کودکان مورد استفاده قرار گیرند. هدف از مطالعه حاضر بررسی تأثیر کاشت حلزون بر شکل پذیری قشر شنوایی با استفاده از CAEPs در کودکان بود. روش بررسی: در این مطالعه مقطعی- تحلیلی تعداد 42 کودک (2-3 سال) با کم شنوایی پیش زبانی عمیق شرکت کردند. پاسخ های CAEPs در مراحل زمانی پیش از کاشت، 4 ماه پس از کاشت و 8 ماه پس از کاشت ثبت گردیدند و زمان نهفتگی و دامنه موج P1 در پاسخ به محرکات گفتاری /m/، /g/، و /s/ تعیین شد. یافته ها: میانگین زمان نهفتگی موج P1 بعد از عمل کاشت حلزون نسبت به پیش از کاشت به صورت معناداری کاهش پیدا نموده بود (p <0.001). دامنه موج P1 نیز برای در زمان بعد از عمل کاشت حلزون نسبت به زمان قبل از آن به صورت معناداری افزایش پیدا کرده بود (p <0.001). بعلاوه، بیشترین میزان کاهش زمان نهفتگی، و بیشترین میزان افزایش دامنه مربوط به محرک /g/ و کمترین آن برای محرک /m/ به دست آمده بود. نتیجه گیری: کاهش زمان نهفتگی و افزایش دامنه پاسخ های CAEPs بعد از کاشت حلزون در پاسخ به محرکات گفتاری بیانگر افزایش فعالیت عصبی (نورو پلاستیسیتی) در کودکانی است که در زمان مناسب تحت عمل کاشت حلزون قرار گرفته بودند.

مقایسه روش سفوکسی تین دیسک دیفیوژن با سایر روشهای فنوتیپی و مولکولی در شناسائی استافیلوکوکوس ارئوس های مقاوم به متی سیلین درنمونه های بالینی بیماران بستری دربیمارستانهای آموزشی شهر اهواز
زمینه و هدف:استافیلوکوکوس ارئوس یکی از معدود باکتریهای قادر به ایجاد عفونت در تمامی ارگان های بدن می باشد. این مطالعه با هدف بررسی مقایسه روش سفوکسیتین دیسک دیفیوژن با سایر روش های فنوتیپی و مولکولی در تعیین استافیلوکوکوس ارئوسهای مقاوم به متیسیلین (MRSA) در نمونههای بالینی بیماران بستری در بیمارستان های آموزشی شهراهواز و بررسی شیوع نسبی و تعیین الگوی حساسیت آنتیبیوتیکی این باکتری طراحی و تنظیم گردید. روش بررسی: تعداد 693 ایزوله استافیلوکوک از نمونه های بالینی بیمارستان های اهواز جمعآوری گردید. با استفاده از روش های فنوتیپی و مولکولی استاندارد 280 سویه به عنوان استافیلوکوکوس ارئوس جداسازی شدند. مقایسه روشهای شناسائی سویههای مقاوم به متیسیلین با استفاده ازOxacillin disk diffusion ، Oxacillin screening agar، Cefoxitin disk diffusion و PCR انجام شد. همچنین تست حساسیت استافیلوکوکوس اورِئوس با روش های دیسک دیفیوژن و ونکومایسین آگار اسکرینینگ نیز انجام شد. یافتهها: از مجموع 693 نمونه، 280 نمونه به عنوان باکتری استافیلوکوکوس ارئوس تایید گردیدند. تعداد سویه های مقاوم به متیسیلین، (2/43%)121 سویه با روش دیسک دیفیوژن اگزاسیلین، (5/43%)122 سویه با روش غربالگری آگار اگزاسیلین و (9/43%)123 سویه با روشهای دیسک دیفیوژن سفوکسیتین و PCR شناسائی شدند. در تعیین الگوی مقاومت آنتیبیوتیکی، بیشترین مقاومت نسبت به اریترومایسین (3/98%)، سیپروفلوکساسین(5/97%)، کلیندامایسین (3/94%)، تتراسیکلین(2/90%)، جنتامایسین (7/83%)، ریفامپین(4/41%) و کمترین مقاومت MRSA نسبت به آنتیبیوتیکهای تیکوپلانین(0%)، لینزولاید (0%)، ونکومایسین(0%) بدست آمد. نتیجهگیری: نتایج نشان دهنده مقاومت نسبتا بالای آنتیبیوتیکی سویههای استافیلوکوکوس ارئوس جدا شده از بیمارستانهای مورد مطالعه می باشد. خوشبختانه مقاومت 0% نسبت به آنتیبیوتیکهای ونکومایسین، تیکوپلانین و لینزولاید وجود داشت. Cefoxitin disk diffusion بهترین روش در تعیین سویههای MRSA می باشد.

زمینه: نوروپاتی محیطی از عوارض شایع دیابت است که منجر به علائم بالینی و تضعیف کیفیت زندگی بیماران میشود. هدف از این مطالعه، بررسی تاثیر تمرین مقاومتی بر علائم بالینی و شاخصهای کیفیت خواب، کیفیت زندگی و خستگی در بیماران مبتلا به نورپاتی محیطی دیابت نوع 2 بود. مواد و روشها: در این مطالعه کارآزمایی بالینی 30 بیمار مبتلا به نوروپاتی محیطی دیابت نوع 2 بصورت تصادفی در دو گروه تجربی و کنترل قرار گرفتند. گروه تجربی سه جلسه در هفته برنامه تمرین مقاومتی شامل 11 حرکت با 80-50 درصد یک تکرار بیشینه، 3-1 ست برای هر حرکت به مدت 12 هفته انجام دادند. قبل و بعد از دوره مداخله علائم بالینی نوروپاتی (امتیاز نوروپاتی دیابتی میشیگان) شاخصهای کیفیت زندگی (پرسشنامه کیفیت زندگی ۳۶ سوالی)، کیفیت خواب (پرسشنامه کیفیت خواب پیتزبورگ) و خستگی (پرسشنامه استاندارد سنجش خستگی) ارزیابی شده و نمونههای خون جمعآوری شد. دادههای به دست آمده با استفاده از آزمونهای تی وابسته و مستقل در سطح معنیداری 05/0 تحلیل شدند. یافته: نتایج حاکی از بهبود معنیدار علائم بالینی نوروپاتی محیطی (MDNS) در گروه تجربی در مقایسه به گروه کنترل بود (05/0>P). همچنین، سطح گلوکز ناشتا و هموگلوبین A1Cبعد از مداخله کاهش معناداری را نشان داد (05/0>P). علاوهبراین، کیفیت زندگی، کیفیت خواب و شاخص خستگی در گروه تجربی نسبت به گروه کنترل به صورت معناداری بهبود پیدا کرد (05/0>P). نتیجهگیری: با توجه به نتایج این مطالعه به نظر میرسدتمرینات مقاومتی مداخله مناسبی برای مقابله با علائم مرتبط با نوروپاتی محیطی دیابت و بهبود کیفیت زندگی در این بیماران است.

بررسی تاثیر شبکه های اجتماعی بر میزان تمایل بانوان به انجام جراحی های زیبایی در ناحیه فک و صورت در شهر اهواز سال 98
زمینه و هدف: با توجه به اینکه جراحی های زیبایی در ناحیه فک و صورت یکی از پرطرفدارترین جراحی ها در کشور می باشند و عوامل مختلفی در گرایش به این نوع جراحی ها دخیل هستند، پژوهش حاضر با هدف بررسی تاثیر شبکه های اجتماعی بر گرایش بانوان به این نوع از جراحی ها در شهر اهواز انجام گرفته است. روش بررسی: مطالعه حاضر مقطعی توصیفی تحلیلی است. حجم نمونه با استفاده از فرمول کوکران 200 نفر محاسبه و نمونه گیری به روش تصادفی طبقه ای انجام شد. جامعه آماری، زنان مراجعه کننده به ده مطب از مطب های خصوصی جراحان فک و صورت و متخصصان گوش و حلق و بینی شهر اهواز بودند که جهت دریافت درمان های زیبایی مراجعه کرده بودند. داده ها با استفاده از پرسشنامه جمع آوری و با نرم افزار SPSS و آزمون T مستقل آنالیز شد. یافته ها: نتایج نشان داد که شبکه های اجتماعی می توانند از طرق مختلف مثل کاهش اعتماد به نفس و افزایش تصور منفی نسبت به چهره، تبلیغات، افزایش فشار اجتماعی بر زنان، ایجاد تمایل به نزدیک شدن به چهره های ایده آل و افزایش مقبولیت چهره ای و جذب جنس مخالف در این شبکه ها ، باعث گرایش به جراحی های زیبایی شوند. نتیجه گیری: با توجه به نتایج این پژوهش می توان گفت شبکه های اجتماعی علی رغم داشتن جنبه های مثبت، جنبه های منفی نیز دارند و می توانند در افزایش تمایل بانوان به انجام جراحی های زیبایی در ناحیه فک و صورت موثر باشند.

ارتباط بین پلی مورفیسم ژن مونوآمین اکسیداز A (MAO-A) با انگیزش به فعالیت بدنی و سطح آن در مردان و زنان سالم
زمینه و هدف: MAO-A آنزیمی است که میانجی های عصبی مانند دوپامین و سروتونین را تجزیه می کند. این تحقیق به ارتباط بین پلی مورفیسم ژن مونوآمین اکسیداز A (MAO-A) با انگیزش به فعالیت بدنی و سطح آن در مردان و زنان سالم پردازد. روش بررسی: یکصد و بیست و دو آزمودنی (میانگین سن 72/5 ± 37/24؛ شامل 55 مرد و 67 زن) پرسشنامه ساماندهی رفتاری فعالیت ورزشی 2 (BREQ-2) جهت بررسی انگیزش فعالیت ورزشی و پرسشنامه فرم کوتاه بین المللی فعالیتبدنی (IPAQ) را برای بررسی سطح فعالیتبدنی کامل نمودند. DNA از نمونه سلولهای CHEEK جمع آوری و مجزا گردید. ژنوتیپ MAO-A با استفاده از روش PCR با پرایمرهای مخصوص ژن شناسایی گردید. یافتهها: نتایج آزمون آنالیز واریانس یک طرفه برای مقایسه نمرات BREQ-2 و گروههای انگیزشی خاص نشان داد میانگین نمرات کل BREQ-2 بین ژنوتیپها تفاوت معنی داری نداشت. همچنین، تفاوت معنیداری در دستههای انگیزشی بین ژنوتیپها وجود نداشت (P >0.05). همچنین نشان داد شد که افراد حامل ژنوتیپ 3.5/3.5 فعالیتبدنی بیشتری در هفته نسبت به دو ژنوتیپ دیگر داشتند، هر چند این تفاوت معنی دار نبود (P >0.05). نتیجهگیری: به طور کلی، مطالعه حاضر رابطه مورد انتظاری بین ژنوتیپ MAO-A و سطح فعالیت بدنی را نشان نداد زیرا هیچ تفاوتی بین میزان فعالیت بدنی بین ژنوتیپ ها وجود ندارد. این نشان می دهد که عوامل ذاتی نقش مهمی در تعیین سطح فعالیت بدنی ایفا می کنند.

بررسی ارتباط بیان پروتـیین تنظیم کننده ی سیکل سلولی cyclin D1 با عوامل پیش آگهی دهنده ی سرطان پروستات
مقدمه تاثیر میزان بیان Cyclin D1 در سرطان پروستات هنوز به صورت دقیق بیان نشده است. مطالعه ی حاضر با هدف ارزیابی ارتباط بیان ایمنوهیستوشیمیایی Cyclin D1 با میزان پیشرفت تومور انجام گرفت. مواد و روش ها در این مطالعه ی توصیفی مقطعی از بلوک های پارافینی نمونه بافتی بیماران مراجعه کننده به بیمارستان های آموزشی امام خمینی (ره)،شهید بقایی وگلستان اهواز،با تشخیص پاتولوژی آدنوکارسینوم پروستات و هیپرپلازی خوش خیم پروستات استفاده شد. اطلاعات بالینی و دموگرافیک بیماران از پرونده ی بیمارستانی استخراج شد. جهت رنگ آمیزی ایمونوهیستوشیمی از آنتی بادی منوکلونال Anticyclin D1 استفاده شد. یافته ها در این مطالعه 69 بیمار مبتلا به سرطان پروستات و 10 بیمار مبتلا به هیپرپلازی خوش خیم پروستات مورد مطالعه قرار گرفتند. میانگین سنی بیماران برابر با 70 سال بود و در رنج 40 تا 92 سال قرار داشتند. ارزیابی بیان ایمنوهیستوشیمیایی Cyclin D1 نشان داد که تنها 6 درصد بیماران به سرطان منفی بوده و 32% نیز بیان بسیار قوی داشته اند. تمامی بیماران مبتلا به هیپرپلازی خوش خیم پروستات هم بیان منفی داشتند. مشاهده شد که بیان شدید(plus plus) مارکر cyclin D1 به طور معنی داری در بیماران High grade بیشتر است(p=0.02). نتیجه گیری به طور کلی یافته های این مطالعه نشان می دهد که مارکر Cyclin D1 به طور وسیعی در بدخیمی پروستات افزایش یافته و همچنین شدت این افزایش بیان می تواند مرتبط با گرید میکروسکوپیک توموربر اساس معیار گلیسون باشد.

بررسی شاخص های کنترل گلیسمیک در مبتلایان به دیابت تیپ 1 تحت درمان با روشconventionalو Basal and Bolus در بیمارستان کودکان مفیدطی سال های 93-97
چکیده: زمینه و هدف: درمان دیابت نوع یک در کودکان با انواع رژیم های درمانی انسولین امکان پذیر می باشد. بررسی و مقایسه فواید و مشکلات هر یک از این رژیم های درمانی می تواند به تصمیم گیری در مورد انتخاب نوع رژیم انسولین در کودکان کمک کند.هدف از این مطالعه بررسی مقایسه ای کنترل گلیسمیک و شاخصهای رشدی در مبتلایان به دیابت تیپ 1 تحت درمان با روش conventional و Bolus and Basal بود. روش بررسی: در این مطالعه مقطعی 73 کودک مبتلا به دیابت تیپ 1 در دو گروه درمانیconventional 35) نفر) وBolus and Basal ( 38نفر) شرکت کردند.. اطلاعات جمع آوری شده شامل سن، جنس، قد، وزن، نمایه توده بدنی، مدت زمان ابتلای کودک به دیابت، میانگین قند های ناشتای کودک در یک ماه اخیر، میانگین قند های خون Post Prandial در یک ماه اخیر،میانگین سه دوره سه ماهه هموگلوبین A1c ، تعداد حملات هایپوگلایسمی در یک ماه اخیر، تعداد دفعات حمله DKA در یک سال اخیر بودند. یافته ها :در این مطالعه Z score قد و وزن بیماران در گروه Basal&Bolus به صورت معنی داری بالاتر از گروه Conventional بود. P: 0.021)) در حالیکه تفاوت معنی داری در مورد Z Score نمایه توده بدنی در دو گروه وجود نداشت.بررسی کنترل گلیسمیک بیماران و بروز عوارض حاد (هیپوگلیسمی وکتواسیدوز دیابتی) تفاوت معنی دار در این دو روش درمانی نداشت. نتیجه گیری: در بیماران ایرانی انتخاب روش درمان در هر فرد مبتلا با توجه به ویژگی های فردی و خانوادگی و سایکولوژیک توصیه می شود.

تعیین میزان آگاهی پزشکان عمومی و متخصصان اطفال شهرستان اهواز (جنوب غرب ایران) در مورد بیماری های نقص سیستم ایمنی اولیه
.مقدمه: نقص های ایمنی اولیه (PID) گروهی از اختلالات هتروژن سیستم ایمنی هستند و در صورتی که به موقع تشخیص و درمان نشوند منجر به عفونت های مزمن و کشنده می شوند. مطالعه حاضر با هدف تعیین میزان آگاهی پزشکان عمومی و متخصصان اطفال شهرستان اهواز از بیماری های نقص سیستم ایمنی اولیه انجام شد. روش کار: یک پرسشنامه در 5 بخش مشخصات بالینی، علائم و بیماری های مرتبط، سابقه خانوادگی، بررسی های آزمایشگاهی و مدیریت بیماران PIDs طراحی و قابلیت اعتماد و اعتبار آن از طریق یک مطالعه پایلوت تائید شد (آلفای کرونباخ= 79/0). نتایج: در مطالعه حاضر 257 پزشک (57 متخصص اطفال و 200 پزشک عمومی) شرکت داشتند. میانگین سنی شرکت کنندگان 54/8 ± 35/30 سال (محدوده 25 تا 59 سال) بود که شامل 48/61 درصد زن و 52/38 درصد مرد بودند. میانگین امتیاز کلی آگاهی متخصصان اطفال 68/10 ± 22/65 درصد (بین 31/49 تا 41/90) و امتیاز کلی پزشکان عمومی 47/5 ± 91/58 (بین 20/45 تا 34/75) بود. اختلاف معناداری در درصد پاسخ های صحیح بین متخصصان اطفال و پزشکان عمومی مشاهده شد (0001/0>P). علاوه براین، 26 نفر (6/45درصد) از متخصصان اطفال و 19 نفر (5/9 درصد) از پزشکان عمومی بیش از دو سوم سوالات (>67% سوالات) را درست پاسخ دادند. نتیجه گیری: نتایج این مطالعه نشان داد پزشکان در شهر اهواز نیاز به آگاهی بیشتری در مورد بیماری های نقص سیستم ایمنی دارند و لذا آموزش های حین تحصیل و پس از فارغ التحصیلی در این زمینه برای پزشکان در اهواز ضروری به نظر می رسد.

مقایسه تاثیر موسیقی درمانی و نوشتن بر بهبود کیفیت زندگی دانش آموزان مبتلا به سرطان ۸ تا ۱۴ سال
زمینه و هدف: سرطان بیماری است که اثرات قابل توجهی بر زندگی افراد مبتلا و خانواده های آن ها بر جای می گذارد. این مطالعه با هدف مقایسه تاثیر موسیقی درمانی و نوشتن بر بهبود کیفیت زندگی دانش آموزان مبتلا به سرطان ۸ تا ۱۴ سال انجام شد. روش بررسی: این مطالعه از نوع نیمه آزمایشی با طرح پیش آزمون - پس آزمون با گروه کنترل بود. تعداد 45 دانش آموز 8 تا 14 سال به روش هدفمند از بخش انکولوژی بیمارستان فوق تخصصی کودکان بندرعباس انتخاب و به طور تصادفی در دو گروه آزمایش و یک گروه کنترل جایگزین شدند. ابزار تحقیق در دو مرحله در دو گروه گردآوری شدند. در گروه موسیقی در 4 هفته شنیدند و گروه نوشتن 4 هفته متوالی به صورت روزی 30 دقیقه نوشتن درﺑﺎره اﺣﺴﺎﺳﺎت اﻓﮑﺎر و ﻫﯿﺠﺎﻧﺎت ﺧﻮد در ﻣﻮرد روﯾﺪاد یا ﺑﺎر ﻫﯿﺠﺎﻧﯽ بیماری سرطان نوشتند. داده ها به کمک آزمون تحلیل کواریانس چندمتغیری با استفاده از نرم افزار spss نسخه 23 تحلیل شدند. یافته ها: نتایج نشان داد بین اثربخشی موسیقی درمانی و نوشتن بر عملکرد جسمانی، عملکرد اجتماعی و کیفیت زندگی تفاوت معنی دار بود. اما تفاوت در تاثیرگذاری دو گروه بر عملکرد تحصیلی و عملکرد عاطفی تفاوت معنی دار نبود. نتیجه گیری: درواقع درمان هایی که به تخلیه هیجانات منفی منجر می شود به بهبود وضعیت سلامت روان آنها می انجامد.

مقایسه تغییرات مرکز فشار و زمان واکنش در شروع راه رفتن در پاسخ به اغتشاش بیرونی زنان مبتلابه مولتیپل اسکلروزیس و سالم
مقدمه و هدف:مولتیپل اسکلروزیس بیماری مزمن التهابی مربوط به سیستم ایمنی که سیستم عصبی مرکزی را تحت تأثیر قرار میدهدومیتواند منجر به تغییر درراه رفتن، عدم تعادل پاسچرال ، محدودیت در فعالیتهای روزانه و افزایش خطر مصدومیت ناشی از افتادن در این بیماران شود. هدف این پژوهش مقایسه تغییرات مرکز فشار، زمان عکس العمل و اختلاف مرکز فشار به جرم حین شروع راه رفتن زنان جوان 20تا 40 سال مبتلابه MS در پاسخ به اغتشاش بیرونی بود. روش شناسی: 10بیمار MS و10 فرد سالم پروتکل شروع راه رفتن را از حالت ایستاده ثابت به حالت یکنواخت راه رفتن روی دو صفحه نیرو انجام دادند. یافته ها: بیماران MS با ماکزیمم فاصله روبهجلو مرکز فشار و تغییرات بیشتر در اختلاف مرکز فشار نسبت به مرکز جرم در جهت روبهجلو و طرفین در مرحله فیدفوروارد و تغییر بیشتر در اختلاف مرکز فشار نسبت به مرکز جرم درحرکت به طرفین در مرحله فیدبک راه رفتن خود را نسبت به افراد سالم شروع میکنند.نتیجه گیری:به نظر می رسدکاهش بیشینه مرکز فشار در هر دو جهت و اختلاف بین مرکز فشار و جرم در مرحله فیدفوروارد در بیماران MS می تواند دلیلی برای ارتباط بین شروع راه رفتن و سقوط ناشی از کاهش زمان واکنش، کاهش کنترل پوسچرال و یا هر دو باشد. از طرف دیگر افزایش اختلاف مرکز فشار و جرم در مرحله فیدبک می تواند دلیلی برای کنترل مداوم مرکز جرم بدن در داخل مرز پایداری تلقی شود که نیاز انجام تمرینات تعادلی در اندام تحتانی بیماران MS را مورد تاکید قرار دهد.
