Non-specific symptomatology in infants and young children makes the clinical differentiation between lower UTI and acute pyelonephritis (APN) difficult. The aim of this study was to assess the correlation between APN findings of renal cortical scintigraphy and selected clinical/ laboratory findings of febrile UTI in infants and children admitted at our center. Subjects and Methods: A prospective study was conducted in 83 infants and young children aged 1 month –8 years hospitalized with febrile UTI in nephrology ward of Abuzar children's hospital. Within the first 5 days after admission, Tc-99m DMSA renal scintigraphy, ultrasonography (US), voiding cystoureterography (VCUG), erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), hemoglobin (Hb), white blood cell count (WBC) and urine analyses were performed. Results: Mean age was 24.3 months with 82% (68) girls. DMSA scintigraphy showed APN findings in 45/83(54.2%) patients, with a mean age of 30.2 months, including 9 males (20%) and 36 (80%) females. There were statistically significant correlations between the APN findings of DMSA scintigraphy and the fever duration, body temperature, lucocytosis, anemia, proteinurea, CRP levels and ESR (p<0.05). Vesicoureteral reflux was found in 20.5% of patients with no statistically significant correlations to the APN findings of DMSA scintigraphy. Conclusion: Although initial DMSA renal scintigraphy is useful for determination and localization of kidney involvement during febrile UTI, some clinical and paraclinical findings can predict the scintigraphycal findings of kidney involvement that need further evaluations for portable complications in the future.}, keywords = {Acute Pyelonephritis,Renal Scar,Vesicoureteral reflux,Proteinurea}, title_fa = {ارتباط یافته‌های سنتی‌گرافی کلیه با یافته‌های بالینی و آزمایشگاهی در کودکان مبتلا به عفونت تب‌دار مجاری ادراری}, abstract_fa = {زمینه و هدف: عفونت مجاری ادراری یکی از بیماری­های شایع و مهم کودکان است. علایم غیراختصاصی این بیماری در دوران شیرخوارگی و در کودکان خردسال، تشخیص عفونت ادراری فوقانی از تحتانی را مشکل می­سازد. هدف از این مطالعه، بررسی رابطه بین یافته­های پیلونفریت در اسکن کلیه با یافته­های بالینی و آزمایشگاهی در کودکان مبتلا به عفونت ادراری تب­دار بوده است. روش بررسی: این مطالعة آینده­نگر بر روی 83 شیرخوار و کودک یک ماه تا 8 ساله که به­دنبال عفونت ادراری تب­دار در بخش کلیة بیمارستان کودکان ابوذر بستری شده بودند، انجام شد. در 5 روز ابتدایی بستری بیماران، اسکن کورتکس کلیه با DMSA نشان­دار شده با تکنسیوم 99، سونوگرافی، VCUG، ESR، CRP، هموگلوبین، تعداد گویچه­های سفید خون و آنالیز کامل ادرار انجام شد. یافته­ها: میانگین سنی بیماران ما 3/24 ماه و 82 درصد (68 نفر) از آنان دختر بودند. اسکن کلیه در 45 نفر از بیماران (2/54 درصد) علایم پیلونفریت حاد را نشان داد که سن متوسط آنان 2/30 ماه و شامل 9 پسر (20 درصد) و 36 دختر (80 درصد) بودند. مدت تب، میزان تب، لکوسیتوز، کم­خونی، پروتئینوری، ESR بالا و CRP مثبت، رابطة آماری معناداری با یافته­های پیلونفریت حاد در اسکن کلیه داشتند (05/0p<). برگشت ادرار به حالب­ها در 5/20 درصد از بیماران مشاهده شد که رابطة معناداری با یافته­های پیلونفریت حاد در اسکن کلیه نداشت. نتیجه­گیری: گرچهبا انجام اسکن کورتکس کلیه در طی ابتلا به عفونت تب­دار مجاری ادراری می­توان درگیری کلیه و قطب مبتلای آن را مشخص نمود، برخی از یافته­های بالینی و آزمایشگاهی قادرند ایجاد علایم پیلونفریت را در اسکن کلیه پیش­گویی کنند و نیاز به بررسی­های بیشتر برای تشخیص عوارض احتمالی در آینده را مطرح نمایند.}, keywords_fa = {پیلونفریت حاد,اسکار کلیه,برگشت ادرار از مثانه به حالب,پروتئینوری}, url = {https://jsmj.ajums.ac.ir/article_49462.html}, eprint = {https://jsmj.ajums.ac.ir/article_49462_5ea165a0b2d0951a7229f459afdf1ebf.pdf} }
