The aim of this study was to compere three different methods of nutritional assessment for evaluation the nutritional status of these patients. Subjects and Methods: In this study, 30 pediatric patients with chronic kidney disease (6 to 20 years) with GFR=15-60 ml/min/1.72m2 were selected and compared with 30 healthy control subjects. Nutritional assessment was made by use of subjective global assessment (SGA), measuring serum albumin, pre-albumin, IGF-I and body composition analysis performed by bioelectric impedance analysis (BIA) method. The data were analyzed using SPSS software, normally distributed variables were expressed as mean ± Standard Deviation (SD) and were compared by Independent Student’s t-test. means were considered significantly different at P<0.05. Results: Based on SGA score none of CKD patients had severe malnutrition. Mean albumin and IGF-1 level, were significantly lower in CKD children. BIA results showed patients with CKD had significantly lower dry lean weight, body cell mass, body mass index and basal metabolic rate. Conclusion: The result of present study showed BIA compared with SGA and biochemical factors is more effective in early detection of malnutrition in CKD children. Therefore this study suggests that BIA can be an attractive clinical tool to detect malnutrition in CKD.  }, keywords = {Protein Energy Malnutrition,Chronic kidney disease,children}, title_fa = {مقایسۀ سه روش مختلف (ارزیابی جامع ذهنی، آنالیز مقاومت بیو الکتریک و فاکتورهای بیوشیمیایی) دربررسی وضعیت تغذیه کودکان و نوجوانان مبتلا به نارسایی مزمن کلیه}, abstract_fa = {زمینه و هدف: یکی از مشکلات شایع در بیماران مبتلا به نارسایی مزمن کلیه Chronic Kidney Disease(CKD)، سوء­تغذیۀ پروتئین- انرژی Protein-Energy Malnutrition (PEM) می­باشدکه باعث کاهش کیفیت زندگی و افزایش مرگ و میر می­گردد. هدف این مطالعه، مقایسۀ سه روش متفاوت ارزیابی وضعیت تغذیه در بررسی وضعیت تغذیۀ این بیماران می­باشد. روش بررسی: در این مطالعه 30 کودک مبتلا به نارسایی مزمن کلیه (دامنۀ سنی 6-20 سال) با GFR  m273/1/min/ml 15-60 انتخاب و با 30 کودک سالم مقایسه گردیدند. وضعیت تغذیۀ این افراد از طریق تکمیل فرم SGA، اندازه­گیری آلبومین، پره آلبومین وIGF-1 سرم و اندازه­گیری ترکیبات بدن با استفاده از روش BIA ارزیابی گردید. داده­های به­دست­آمده با استفاده از نرم­افزار SPSS آنالیز شد.متغییرهای دارای توزیع نرمال به صورت میانگین ± انحراف معیار گزارش و توسط آزمون student’s t-test مقایسه گردیدند. اختلاف آماری با 05/0 P< معنادار در نظر گرفته شد. یافته­ها: در این مطالعه بر مبنای روش SGA، هیچ­یک از بیماران، سوء تغذیۀ شدید را نشان ندادند. میانگین سطوح آلبومین و IGF-1 سرم در بیماران به­طور معناداری کمتر از گروه شاهد بود. همچنین بیماران سطوح پایین­تری از Dry Lean Weight، Body Cell Mass،Body Mass Index و Basal Metabolic Rate  را در مقایسه با گروه شاهد نشان دادند. نتیجه­گیری: یافته­های مطالعۀ حاضر پیشنهاد می­کند که روش BIA از نظر تشخیص  PEMدر مراحل اولیه CKD، در مقایسه باSGA و ارزیابی بیوشیمیایی مفیدتر می­باشد و می­توان از آن به­عنوان یک ابزار کاربردی بالینی در تشخیصPEM  استفاده کرد}, keywords_fa = {سوء تغذیۀ پروتئین- انرژی,نارسایی مزمن کلیه,کودکان}, url = {https://jsmj.ajums.ac.ir/article_49136.html}, eprint = {https://jsmj.ajums.ac.ir/article_49136_323c61881b220d60678cbeca1623d89a.pdf} }
