It has an important role in enhancing the diagnostic accuracy of skin lesions. The aim of this study is evaluation of the use of dermoscopy in diagnose of malignant skin lesions and premalignant skin lesions. Methods: This prospective observational study conducted on 240 patients with clinical diagnosis of malignant or pre-malignant skin lesions. All the patients were evaluated with dermoscopy and histopathology. Dermoscopic results were compared with histopathologic findings as the gold standard for diagnosis of skin lesions. Findings: Of the 240 skin lesions, 69 (28.8%) were benign, 75 (31.3%) were pre malignant and 96 (40.0%) were malignant. The concordance between dermoscopic results and histopathological findings was 75.4%. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of dermoscopy in diagnosis of malignant lesions were 85.42%, 70.59%, 79.27%, 80.40%, 77.42% and 79.27% respectively, p < 0.0001. These items for pre-malignant lesions were 66.67%, 70.59%, 71.43%, 65.75% and 68.54% respectively. Discussion: The sensitivity and specificity of dermoscopy in diagnosis of skin lesions were high, therefore this real-time non-invasive technique can be introduced as a routine diagnostic tool in dermatological examination and can minimize unnecessary invasive biopsy.}, keywords = {Dermatoscopy,Skin lesion,histopathology,Diagnostic accuracy}, title_fa = {ارزیابی کاربرد درموسکوپی در تشخیص بالینی ضایعات پوستی بدخیم و ضایعات پوستی پیش بدخیم}, abstract_fa = {مقدمه: درموسکوپی یک روش تشخیصی غیرتهاجمی است که اجازه مشاهده ویژگی‌های مورفولوژیکی که با چشم غیرمسلح قابل تشخیص نیست را فراهم می‌کند. درموسکوپی باعث بهبود قابل توجه در دقت تشخیصی ضایعات پوستی شده است. هدف این مطالعه ارزیابی کاربرد درموسکوپی در تشخیص بالینی ضایعات پوستی بدخیم و ضایعات پوستی پیش بدخیم می‌باشد. روش‌ها: این مطالعه مشاهده ای آینده نگر بر روی240 بیمار با ضایعات پوستی مشکوک به بدخیمی و پیش بدخیمی انجام شد. تمام بیماران تحت ارزیابی درموسکوپی و هیستوپاتولوژیک قرار گرفتند. نتایج درموسکوپی با یافته های هیستوپاتولوژیک به عنوان استانداد طلایی تشخیص مقایسه شد. یافته‌ها: از میان 240 ضایعه پوستی مورد بررسی 69 مورد (8/28 %) خوش خیم، 75 مورد (3/31 %) پیش بدخیم و 96 مورد (0/40 %) بدخیم بودند. توافق بین درموسکوپی با نتایج بیوپسی 4/75 % بود. حساسیت، ویژگی، ارزش اخباری مثبت، ارزش اخباری منفی و دقت درموسکوپی در تشخیص ضایعات بدخیم به ترتیب برابر 42/85 %، 59/70 % ، 40/80 %، 42/77 % و 27/79 % می‌باشد؛ این موارد برای ضایعات پیش بدخیم به ترتیب برابر 67/ 66 %، 59/70 %، 43/71 %، 75/65 % و 54/68 % بود. بحث: نتایج این مطالعه نشان داد درموسکوپی یک ابزار ارزشمند برای تمایز ضایعات پوستی مختلف می‌باشد. همچنین حساسیت و ویژگی بالای درموسکوپی به ویژه در تشخیص ضایعات بدخیم نشان می‌دهد درموسکوپی می‌تواند به عنوان یک ابزار تشخیصی روتین در بررسی‌های درماتولوژیک استفاده شود و قبل از انجام بیوپسی تهاجمی پوست، کمک زیادی به تشخیص دقیق ضایعات پوستی کند.}, keywords_fa = {درموسکوپی,ضایعات پوستی,هیستوپاتولوژی,دقت تشخیصی}, url = {https://jsmj.ajums.ac.ir/article_120315.html}, eprint = {https://jsmj.ajums.ac.ir/article_120315_5c9b5fb01d68095f06536fb1305561cc.pdf} }
